Incident Report Form For reporting leaks, spills, fires, etc.

Incident Report will be shut down from May 25, 2017, at 4:00 p.m. (MDT), until June 1, in preparation for switching to our new database.

Operators can submit electronic reports of undesirable events (oil and natural gas leaks, spills, fires, etc.) to the division. To know more, click here.

NOTE: Operators may now amend existing reports previously entered into the system. Original reports should be amended when corrections are needed, rather than creating new reports for the same event.

This program allows oil and gas operators to electronically submit Applications for Permit to Drill (APD), Sundry Notices, Well Completion Reports, and Workover Tax Credit applications to the Utah Division of Oil, Gas and Mining.

PDF WELL LOGS MAY NOW BE SUBMITTED ONLINE  
PDF copies of well logs are now being accepted online. Operators may submit such logs via our ePermit program through the PDF Well Log option (on the main menu) or as attachments to a Well Completion Report. Operators submitting their well logs as PDF images will no longer be required to submit paper copies of their logs to the Utah Division of Oil, Gas and Mining.


A registration form (below) must be submitted prior to using this system.

If you encounter problems with ePermit or have questions, please contact us via email at OG-ePermit@utah.gov. Please include the number of the APD or Sundry you are working on, if available.

A company must register to use the ePermit system by completing this form and mailing it to the Utah Division of Oil, Gas and Mining prior to electronically submitting Applications for Permit to Drill (APD), Sundry Notices, or Workover Tax Credits. The original, notarized form must be submitted. Copies will not be accepted.

- Oil and Gas Production Reports (Form 10),

- Gas Plant Reports (Forms 13A & 13B),
- Injection Reports (UIC Forms 2 & 3)
